Advance care planning - making plans for your end of life care and death
Dying is an inevitability we will all face. It is a topic most of us shy away from discussing, but talking about it can make all the difference to the person reaching the end of their life and those around them.
‘My Future Care Plan’ has been designed by patients, carers and health care staff to help people to start having those difficult conversations and to put their affairs in order. It has been jointly published by the CCG and Northern Lincolnshire and Goole NHS Foundation Trust (NLaG).
The plan has been designed to help patients think about the type of care they wish to receive in their final days. This could be decisions about whether they would want to be cared for in their own home or in hospital, who they would like to make decisions for them if they are unable to and what kind of funeral they would like.
Although aimed at end of life patients, the document can be used by anyone to document their wishes.
